Bipolar RF places both the active and return electrodes on the same handpiece, a short distance apart. The current flows only through the tissue directly between them, which keeps the heated zone shallow and tightly controlled.
The advantage is predictability and safety: because the energy path is confined, the operator knows exactly where the heat is going, with less reliance on a return pad and less risk of unintended deep heating. The limitation is depth—bipolar alone reaches less far than monopolar RF. This is why bipolar configurations are often paired with vacuum (to draw tissue into the electrode field) or combined with other depths, and why bipolar is the basis of many RF microneedling systems, where the needles place the energy at a precise dermal depth.
